Nuerenberg Germany helps Kalmunai

In March this year, a Community Health Centre, the Heinrich A Litzenroth Memorial Health Centre was opened in Sainthamaruthu, Kalmunai. The two storyed, 550 sq metres building built at a cost of Rs 23 Million will provide the basic outpatient medicalcare to approximately 30,000 people living in Sainthamaruthu. At the same time it will also serve as an advisory clinic on hygienic, maternity health and nutrition to the community.


Dr Fahmy Ismail, National Technical Advisor, UN Habitat and Markus Spring from the City Council of Munich, Germany make a presentation of medical equipment valued at over Rs 3.5 million donated to the Asroff Memorial Hospital staffby GfK Group, Neurenberg in the presence of Medical Superintendent Dr A L F Rahman, and Dr Ilahi, MOH

City Council of Nuerenberg in association with UN-Habitat and the City of Munich selected Sainthamaruthu, Kalmunai as the site for a Health Centre on a request made by Kalmunai Municipal Council. Kalmunai has a population of over 100,000 and the township was severely damaged by the tsunami in 2004. Sainthamaruthu, one of the divisions in Kalmunai, is a densely populated area with a population of nearly 26,000 and density of 2,777 persons per sq km. The high population density spread in a narrow strip of land in the coastal area was the reason for the high death rate reported at Sainthamaruthu during the tsunami. The livelihoods of most people in the areas depend on fishing and small scale farming.

The Health Centre will meet the needs of a large population in Sainthamaruthu, especially women and children. The centre which is in operation now has placed its primary focus on disease prevention healthcare and pre natal care.

Funds for the Health Centre were donated mainly by the GfK Group in Germany, which is the fourth largest market research organization in the world. It operates in more than 100 countries.

The centre is named after Heinrich A. Litzeroth, managing director and Board Member of GfK, who became a victim of the December 2004 tsunami while holidaying in Khoolath, Thailand.

The projects were coordinated through UN-HABITAT office in Sri Lanka. Dr Fahmy Ismail, National Technical Advisor, UN-HABITAT said that this was an urgent need.

During their visit to Sri Lanka the delegation from Nuerenberg headed by Dr Norbert schurgers, Director of the Bureau of International Relations of the City of Nuerenberg representing the Mayor and Dr Ulrike Schoneberg representing the GfK Group and as Head of the Nuerenberg Sri Lanka Aid Association also visited Ashroff Memorial Hospital. They provided equipment worth of Rs 3.5 Million to the Special Care Baby Unit in the Hospital. The equipment included a defibrillator, patient monitors, infusion pumps, syringe pumps and paediatric stethoscopes.

They also provided funds for the basic equipment including examination beds, instrument/medicine trolley, cupboards, wheel chairs etc, and over 100 chairs. In addition to this they installed play equipment such as see saws, math slide and swings for the children.

The GfK Group has also agreed to fund a proposal that has been submitted by the Kalmunai Municipal Council to address the major issue of flooding caused by the absence of a proper drainage system.

Members of the Association and their guests visit Sri Lanka regularly. The Association has also agreed to conduct an annual Health Camp and provide assistance to preschools and meet educational needs of the needy

3.1) Introduction to the Project:

The Ashraff Memorial Hospital is a leading developing tertiary health care institution in the Regional Director of Health Services division Kalmunai, in the coastal area of the Ampara District in Sri Lanka. This hospital is serving to a population of nearly half million. It is the mother institution to 38 smaller healthcare institutions which come under the purview of the Kalmunai RDHS. Patients are referred to Ashraff Memorial Hospital Kalmunai for specialized care and treatment by these 38 smaller institutions.

This hospital which has accommodation for about 300 beds gives specialized treatment in Medicine, Surgery, Gynaecologys & Obstetrics and Paediatrics and is planning to introduce sub specialarities like Dermatology, Eye, ENT and Radiology etc. in order to extend its services to the patients coming for treatment which is increasing daily. We also have a mobile clinical team which visits remote areas and conducts clinics for the local population. There is an Intensive Care Unit and Premature Baby Care Unit functioning in this hospital. Although we have the above mentioned facilities yet we do not have a Trauma unit which is very essential for management of Road Traffic accident and Home accidents. Further we are unable to meet the medical needs of the ever increasing. Elderly population of this area, especially in Orthopedics and Surgical care.

At present the surgical teams are sharing the Gynaecologys & Obstetrics operation theatre thereby the surgical operative time as well as the intake of patients is restricted. Due to this drawback causes unnecessary transfer of patients, prolonged stay of patients in wards awaiting surgical treatment and long waiting list for elective operations. Further we are unable to maintain adequate sterlity in the Operation Theater

Presently the surgical patients are sharing the Medical Intensive Care Unit with Medical Patients. Therefore a separate Surgical Intensive Care Unit will facilitate better care for surgical patients (every year 50 to 60 patients with critical burn injury are treated.) these patients have to under go prolonged critical care. Therefore a separate Burns Unit will enable better care.

3.2) Specific Problem to be Addressed by the Project:

The trauma Unit is to accommodate Accident service Operating Theatres, Surgical Wards, Eye Ward, ENT Ward, Burn Unit, SICU, Paediatric Surgical Ward and Physiotherapy unit. Providing separate surgical OT will reduce the waiting list for elective surgery, reduce unnecessary transfer of patients to other hospital and enable to maintain good sterile condition. This also improves Gynaecologys & Obstetrics operative time & other operative facilities.
( Last year the total operations done were about 2600-2700 and Gynaecologys & Obstetrics about 1500-1600. This numbers are increasing every year)

3.3) Mode of Intervention in term of this Project (Justification):

Patients seeking treatment for surgical problems and casualty admission following Road Traffic Accidents, Bomb blasts and War casualties are very high in this hospital. At present, there are no proper ward facilities for surgical casualty admissions and orthopaedics admission in this institution. At the same time, this project will bring the Health Care services to the doorstep of poor remote villagers living in this region. This is the main referral centre for Tertiary Care Services for more than 35 smaller institutions in the coastal region of Ampara District with all other supportive facilities such as Intensive Care Unit, Blood Bank and Radiology Unit without adequate infrastructure facilities.

Therefore, this project will definitely improve the health status of the population of this are who were internally displaced and resettled by one or other incidences during the last two decades due to communal violence and civil War and recently by the Tsunami tidal wave. Hence, this project is urgently needed and should be given priority considering the difficulties faced by the poor and marginalized people living in this region.

Ashraff Memorial Hospital is the leading institution in the Deputy Provincial Director of Health Services division Kalmunai, in the coastal area of Ampara District. This hospital is serving to a population of 412542 in this division. It is a mother institution to 35 smaller institutions in the Kalmunai DPDHS area and these 35 smaller institutions are referring patients for specialized care to Ashraff Memorial Hospital Kalmunai.

Ashraff Memorial Hospital is functioning with 262 beds. During the year 2005, there were 87252 patients were treated at Out Patient Department (OPD), 36111 patients were treated at Clinics, 6650 patients were treated at Dental Unit and 19669 patients were treated inwards. There are 10 wards, ICU, Operating Theatre, Labour ward and PBU. This hospital has all supportive facilities like Blood Bank, Radiology unit, Laboratory, and ECG facilities as well. The number of patients seeking treatment at this hospital had been markedly increased in the recent past due to the peace prevailing in the country and recent Tsunami tidal wave which defunct six of close by District Hospital completely. Therefore, this institution is unable to meet the current demand for patient care services due to inadequacy of infrastructure facilities particularly inward facilities for surgical units.

In the recent past the number of Road Traffic Accidents, Bomb Blast injuries and civil war casualties seeking admission to this hospital have been remarkably increased. Therefore the demand for the well equipped Accident & Emergency Unit and Surgical Casualty wards for providing life saving measures and follow up proper patient management facilities felt mandatory.

In order to meet the growing demand our consulting are working hard to manage with limited bed strength, it was pointed out to be having the highest bed occupancy ratio of over 100% with equally highest bed turn over rate of 24% for the 3rd quarter of 2005 hospital bulletin of line ministry hospital.
The following patient care services are scheduled to commence in this institution in near future.

It is proposed to construct a four-storied building to accommodate ground floor Matron Family Suit and Sister / Master family suit, 2nd & 3rd Floor Nurses quarters with attached bathroom single units and the fourth floor Midwives quarters with attached bathroom considering the availability of land for future development of this hospital.

The propose building will be a four-storied building of 130 x 76 size with one elevator and two staircases.

At present, there are no quarter’s facilities for Nursing Officers, Midwives, Paramedics and other category of staff in this hospital. There are 54 Medical Officers, 126 Nursing Officers, 15 paramedical staffs, 65 Minor Employees and 08 Ambulance Drivers are working at present. More than 70% of the staffs are outsider and none of them are provided with a reasonable quarters/residence facilities to work mentally peacefully.

If quarter’s facilities are provided to staffs working in this hospital, they will be residing within the hospital premises and available for duty round the clock. Construction of Quarters Complex will prevent unnecessary delay in getting quality healthcare services for the people of this region. Hence this project is needed and should be given priority considering the difficulties faced by the poor and marginalized people living in this region.
